Medicare Payable Health Equity Services for Community Health Workers (CHWs)

The Centers for Medicare & Medicaid Services (CMS) has released the 2024 Medicare physician fee schedule which outlines a framework for health equity. They made provisions for coverage and payment for four types of services for health-related social needs: caregiver training, social determinants of health risk assessment, community health integration, and principal illness navigation. This webinar will provide a brief overview of this new framework for health equity and how CHW services may be impacted.

(2) Identify the new billable services that address the health equity framework.

(3) Describe the documentation needed to ensure payments.
