Training for Community Health Workers in Primary Healthcare

The South Carolina Primary Health Care Association (SCPHCA) was a recipient of a 2020 UnitedHealthcare Empowering Health Grant which is provided to expand access to care and address social determinants of health for underserved populations. In order to accomplish this goal, the SCPHCA is seeking to increase and expand utilization of the CHW model to facilitate more comprehensive, high quality community-based care across South Carolina.  The SCPHCA will be partnering with the Center for Community Health Alignment to provide 20 community health center staff with Community Health Worker Core Competency training aimed at reducing social isolation in vulnerable populations living in underserved communities. Additional training for 20 community health center supervisors will be provided to help leadership better support their community health worker staff in their new roles.
