Understanding Chronic Disease – April 11 & 12 (9 am – 11:30 am EST)
Supporting Participants Living with Diabetes – April 15 – 19 (9 am – 12 pm EST)
This training curriculum is designed for CHWs and health professionals working within similar roles. It is offered in two parts. The first component (5 hrs) provides learners with an introduction to understanding chronic disease. This introduction module encompasses topics such as: obesity, overweight, BMI, chronic & non-chronic conditions, infectious & non-infectious conditions, and an overview of prediabetes, type I, type II, and gestational diabetes. The second, or advanced, (15 hrs) component of this training provides a more in-depth review of supporting clients living with diabetes. We will cover healthy behaviors to assist with chronic disease management as it relates to health coaching.

We also offer a gestational diabetes module in addition to the introductory and advanced training components. Facilitation of this module is based on expressed demand.
